Navigate to the AMOE page. Add your email and click Continue. On the next page, click Generate Code and your 11-digit code will appear — screenshot it or write it down, you'll need it on your mail-in card.

Each code can only be used once.
Only codes generated through the AMOE page on our website are valid.
